The School-Based Health Team from Akron Children's Hospital will be in Triway on Wednesday, September 25, 2024. They will offer the following vaccines for eligible students 11 years-old and older:

Participation is voluntary. Only students with a parent/guardian completed consent form can receive vaccinations. Paper consent forms will be available in the office. Complete the consent form and select the vaccines your child needs. Return the consent no later than Friday, September 13th. Consents are also attached below.
